Conditional Form using WS Forms

This form is built using a new form manager. It includes conditional logic. Mandatory entries are included in the form depending on whether the visit occurred or not.

It means there is less to fill in.

There are drop down selections for dates and times. Times are in 5 minute increments.

A copy of the form is automatically sent to the volunteer

The co-ordinator can also reply back to the sender in this form

This is not your current volunteer form, but if you want this version live I can replace the Contact Form 7 version of it.
